- is under the care of a physician and skilled nursing is required
- needs teaching on new medication
- may need administration / teaching on insulin
- requires monitoring / teaching on exacerbation of disease process
- may need dressing changes or wound care
- may need physical, occupational, and/or speech therapy
- may need an MSW to evaluate home environment and utilize community services available
- needs home health aide assist with activities of daily living
- needs lab draws for diagnostic / monitoring
- will receive an overall increase in quality of life for the patient
